Quick update: I recently partnered up with three other doulas- Dalia Abrams, Dana Gale, and Ellen Kovalsky- to form the new BirthWell in Birmingham. We are taking clients on an individual basis but are dedicated back-up for each other. This means that any of our clients is guaranteed to have one of us as her doula no matter what! Even better, clients who take either my Hypnobabies class or Dalia’s Lamaze class will receive a discount when they hire a BirthWell in Birmingham doula!

I have openings for one more client in October, two in November, and guess what? I will be IN TOWN (as will the rest of our group) in December for all you moms looking for December doulas . Call me or e-mail me for more information!

Hypnobabies, My Baby, Your Babies!

Alabama’s first full length group Hypnobabies Class series will start Saturday, July 9th! I’ll be teaching at BUCO- 3008 Clairmont Ave- from 9:30-12:30. Hypnobabies is an amazing, unique six week childbirth preparation program. Second order of business- we welcomed our baby girl, Mirabelle Aibhlinn Naylor, into the world (VBAC!!!!) at 4:51am on May 23rd. She was 8lbs 8oz and 19 inches long- and had her hand by her head when she was born!

I will be taking clients on a limited basis starting this fall, so contact me mamas!!

Exciting news!

I actually have a few exciting things coming soon! One is that I am currently working on my certification to be a Hypnobabies Childbirth Hypnosis Instructor!! I completed the four day training in St. Louis on March 7th and I have finished most of the exam. As soon as I flesh out my Hypnobabies-friendly local resources I will be e-mailing off my exam- fingers crossed that I score well enough to pass! I will be the first HCHI in Alabama state-lines and am absolutely thrilled to be bringing this AMAZING class to our area. Seriously, Hypnobabies is awesome.

Even with my eagerness to start teaching classes ASAP, I won’t be able to offer them until later in the summer at the earliest because- drum roll- I will be giving birth to my own bundle of joy sometime in May . We are expecting a baby girl this time. I am so looking forward to having a healing VBAC after my c-section with Davy. But I also know that no matter what the outcome of this birth is, I am prepared and have surrounded myself with the most supportive people and careproviders possible. Whatever decisions made during this birth will be mine and made for the benefit of me and my family.

I will post here when I have more information about my new Hypnobabies blog, Facebook page, and Twitter account, as well as when I will start offering classes and taking clients again!

I’m wishing all expectant and soon-to-be expectant families out there a wonderful pregnancy and birth experience in 2011! For those of you in the Birmingham area, start the new year off right by attending the January meeting of Birmingham Birth Stories and More on Tuesday, January 11th. The topic will be “Expanding your birth mindset: what’s normal vs. what’s common.” In the spirit of expanding how we think of normal labor, check out this recent article about a shift in labor patterns since the 1950s.

That I will be partnering with local artist and photographer Ashley Campbell to provide a whole new level of service for new families! Starting in November, any client that signs a contract (and perhaps even retroactively to anyone who signed in October) gets a free one to two hour newborn photo session with Ashley as well as one free 8 x 10 print. Ashley also offers a variety of other products including birth announcements. Her new site is a work in progress, but you can see a tiny fraction of some of Ashley’s work here. She hopes to add lots of new family pictures soon!

ACOG Releases New VBAC Guidelines!

Today the American College of Obstetricians and Gynecologists (ACOG) released new, less restrictive guidelines concerning candidates for and the support of VBAC- and they are very encouraging! Read about it here.

Also read the International Cesarean Awareness Network’s (ICAN’s) response!

If the majority of doctors and hospitals will subscribe to this long-overdue revision, we may see both a rise in the VBAC rate (and complementary decrease in the cesarean rate- hooray!) and an increase in patient autonomy, which has been severely lacking in many regions in the past decade or so.

Between the Needles and Nightfall

I heard an interview with this musician, Marco Benevento, on NPR a week or so ago. His newest album is called “Between the Needles and Nightfall.” He and his wife had a midwife attended homebirth at which his wife was “induced” with acupuncture early in the morning. The midwife said that she expected the baby would come sometime “between the needles and nightfall” (their daughter arrived just before nightfall) which struck Marco as being poetic. I can’t say I disagree . Though there are aspects of birth- bodily fluids, grunting, squatting just to name a few!- that in and of themselves seem less than poetic- rather base even- in truth all the components of birth work together so perfectly and so beautifully that I think we could be completely justified in calling it the ultimate poetry of the natural world. So, for your consideration and enjoyment, I give you “Between The Needles & Nightfall.”

ABSTRACT

Background Trial of vaginal birth after Caesarean (VBAC) is considered acceptable after one caesarean section (CS), however, women wishing to have trial after two CS are generally not allowed or counselled appropriately of efficacy and complications.

Objective To perform a systematic review of literature on success rate of vaginal birth after two caesarean sections (VBAC-2) and associated adverse maternal and fetal outcomes; and compare with commonly accepted VBAC-1 and the alternative option of repeat third CS (RCS).

Search strategy We searched MEDLINE, EMBASE, CINAHL, Cochrane Library, Current Controlled Trials, HMIC Database, Grey Literature Databases (SIGLE, Biomed Central), using search terms Caesarean section, caesarian, C*rean, C*rian, and MeSH headings ‘Vaginal birth after caesarean section’, combined with second search string two, twice, second, multiple.

Selection criteria No randomised studies were available, case series or cohort studies were assessed for quality (STROBE), 20/23 available studies included.

Data collection and analysis Two independent reviewers selected studies and abstracted and tabulated data and pooled estimates were obtained on success rate, uterine rupture and other adverse maternal and fetal outcomes. Meta-analyses were performed using RevMan-5 to compare VBAC-1 versus VBAC-2 and VBAC-2 versus RCS.

Main results VBAC-2 success rate was 71.1%, uterine rupture rate 1.36%, hysterectomy rate 0.55%, blood transfusion 2.01%, neonatal unit admission rate 7.78% and perinatal asphyxial injury/death 0.09%. VBAC-2 versus VBAC-1 success rates were 4064/5666 (71.1%) versus 38 814/50 685 (76.5%) (P < 0.001); associated uterine rupture rate 1.59% versus 0.72% (P < 0.001) and hysterectomy rates were 0.56% versus 0.19% (P = 0.001) respectively. Comparing VBAC-2 versus RCS, the hysterectomy rates were 0.40% versus 0.63% (P = 0.63), transfusion 1.68% versus 1.67% (P = 0.86) and febrile morbidity 6.03% versus 6.39%, respectively (P = 0.27). Maternal morbidity of VBAC-2 was comparable to RCS. Neonatal morbidity data were too limited to draw valid conclusions, however, no significant differences were indicated in VBAC-2, VBAC-1 and RCS groups in NNU admission rates and asphyxial injury/neonatal death rates (Mantel–Haenszel).

Conclusions Women requesting for a trial of vaginal delivery after two caesarean sections should be counselled appropriately considering available data of success rate 71.1%, uterine rupture rate 1.36% and of a comparative maternal morbidity with repeat CS option.
